 Service Delivery

The Hawkesbury- Hills Division of General Practice has adopted the Practice Support Model of Service Delivery to support General Practice.

The Practice Support Model is designed to promote a coordinated team approach for the support of general practitioners, nurses, practice managers and administration staff.

This "whole practice" approach incorporates the distribution of information and resources on the request of the practice or by suggestion from the practice support officer.

To ensure good relations between the division and your practice we have divided the total number of practices into three geographically defined areas. Each geographical area has a designated Practice Support Officer of which incorporates an average of 25 – 30 practices each.

Your practices "Practice Support Officer" are responsible for being the main contact for your practice in the delivery of division support services. This is achieved by your Practice Support Officer being responsible for the delivery of a number of programs along with increasing your practice’s awareness of other support services and or programs the Division can assist you with.

Examples of how your practice support officer can supporting or assist your practice with are Cold Chain Management, understanding new initiatives, Enhanced Primary Care Items and Assistance with General Practice Quality Improvement measures such as General Practice Accreditation and Clinical Data Management tools. Setting up along with tips to maintain effective recall and reminder systems for your diabetes population is another example of how your practice support officer can provide assistance to your practice.
