Hawkesbury-Hills Division of General Practice (HHDGP) is pleased to announce that we have chosen to participate in the next Virtual Wave of the Australian Primary Care Collaboratives (APCC). The Virtual Wave will allow practices from HHDGP to participate in the APCC quality improvement program at local venues, while also providing opportunities to share knowledge and ideas with practices from across the country.
 
To date, more than 60 Divisions and over 790 Australian general practices have achieved significant improvements through their active participation in the APCC program. By December of this year, 7 practices from HHDGP will have completed their journey in the APCC program. Participating practices have achieved results such as:
 
ü      Improved patient outcomes through better management of diabetes and coronary heart disease
ü      Increased performance through better use of information systems (clinical and billing systems)
ü      A shift from reactive individual patient care to proactive population based care
ü      Changes in service delivery to improve efficiency within the practice
ü      Increased use of protocols and procedures to improve practice operations
ü      Enhanced clinical reporting and functionality (i.e. data cleansing and disease registers)
ü      Improved working environment for clinicians and practice staff.
