The Palliative Care Needs Assessment Guidelines and the Needs Assessment Tool: Progressive Disease – Cancer (NAT: PD-C) were developed to facilitate needs based care for people with advanced cancer and their families, including palliative care. The Guidelines provide a comprehensive review of the physical and psycho-social issues that may impact upon people with advanced cancer, their caregivers, families and health professionals. Used in both generalist and specialist settings, the NAT: PD-C can assist health professionals in matching the types and levels of need experienced with the most appropriate people or services to address those needs.
