Integration
Hawkesbury-Hills Division of General Practice works closely with a range of health services to improve integration between General Practice and other health providers.
This includes:
Facilitating the Hawkesbury District Health Service – GP Liaison Committee.
Through a Memorandum of Understanding with Sydney West Area Health Service (SWAHS), working together to identify common health priorities and working together to improve health care.
Working with local health services to improve information sharing and exchange with General Practice.
The Division publishes a directory of local health services. This includes information about GPs, Specialists, Allied Health Professionals and Community organisations. This is available on-line and in hard copy to local GPs. Several special interest service directories are published and made available to relevant health providers.
