Lifestyle modification programs are an integral component of the broader National Prevention of Type 2 Diabetes Program initiated earlier in 2008 as part of the ‘Council of Australian Governments – Reducing the Risk of Type 2 Diabetes’ initiative.
The Commonwealth Government introduced MBS Item 713, the Type 2 Diabetes Risk Evaluation, to support general practitioners to address the health needs of people aged 40 -49 years who are at ‘high risk’ of developing Type 2 Diabetes.
Reset your Lifeis a nationally accredited and subsidised healthy lifestyle program, or Lifestyle Modification Program (LMP), that aims to reduce the risk or delay the onset of Type 2 Diabetes in participants.
RESET your Life was developed for the Australian General Practice Network (AGPN) by experts from the Baker IDI Heart & Diabetes Institute with financial support of the Commonwealth Department of Health & Ageing. The program meets the Council of Australian Governments Draft Guidelines for Lifestyle Modification Programs for Diabetes Prevention.
Patients aged 40-49 years (inclusive) who are assessed as high risk of developing type 2 diabetes and for whom an Item 713 is claimed. “High risk” is determined by a person scoring 15 or more when they complete the AUSDRISK (the Australian type 2 diabetes risk assessment tool).
The Australian Type 2 Diabetes Risk Assessment Tool (AUSDRISK) has been developed by the International Diabetes Institute (IDI) and is based on the Finnish ‘risk test’ but has been adapted for the Australian clinical setting using AUSDIAB data. It has been developed to provide a basis for both health professionals and health consumers to assess the risk of developing type 2 diabetes over a 5 year period. It consists of a short list of questions that, when completed, provide a guide to a patient’s current level of risk of developing type 2 diabetes.
The completion of the AUSDRISK Tool and a high score result is mandatory for the patient to access Medicare Item 713. It may be completed by either the patient or with the assistance of a health professional or practice staff. To download a copy of the AUSDRISK Tool, click here

The purpose of this Item is to support GPs to address the health needs of patients 40-49 years of age (inclusive) who are at high risk of developing type 2 diabetes. High risk is a score of 15 or more on the AUSDRISK Tool.
Item 713 provides the GP with the opportunity to review a patient’s risk factors and initiate early interventions such as referral to a subsidised Lifestyle Modification Program to reduce the risk or delay the onset of Type 2 Diabetes.

Patients may also be referred to a subsidised LMP by their GP through Item 717 (45-49 year old Health Check) or Item 710 (Aboriginal and Torres Strait Islander Adult Health Check).
Completion of the AUDRISK is mandatory for all patients prior to being referred to this subsidised LMP and must have been completed within the 3 months prior to the Type 2 Diabetes Risk Evaluation service being undertaken.

Patients referred to Reset your life will be required to attend seven group sessions which are held over a 6 – 8 month period with an initial “intensive” phase” lasting 4 months. There will be at least 8 hours of contact time; patients will receive face to face delivery of education & information and the sessions will be interactive. Groups are limited to 15 participants and each session is facilitated by accredited, experienced allied health professionals.
Each patient will receive a “participant’s manual” which is divided into sections based on the each of the seven sessions in the program.
The Reset your life program consists of a series of group motivational and educational sessions supporting lifestyle and behavioural changes and adoption of healthy lifestyle choices in participants. Group sizes are limited to 15 participants. Sessions include:
- The risks of diabetes and their relationship to lifestyle risk factors;
- The importance of regular diabetes screening;
- Nutrition advice and education;
- Physical activity advice and education;
- Behavioural strategies to support the adoption and maintenance of healthy lifestyle changes;
- Smoking cessation and alcohol reduction advice if required; and
- Information about community resources to provide support in maintaining lifestyle change.

RESET your Life is subsidised by the Australian Government and patients will only need to pay a one-off $50 payment for the program. Patients with a health care card or a concession card won’t be asked to pay anything.
